Service Description

FISIOTOOL SOFTWARE LLC provides a technology platform ("Sentinel Health AI") for medical invoice auditing and dispute resolution services.

The Company does not provide medical services, medical advice, diagnosis, or treatment.

Service Fees and Payment

Subscription Plans

Essential: $9.99 per month for individual users
Corporate Pro: $4.99 per employee per month
Success Model: 15% of verified savings only

Important Notice:

For the Success Model, the Company charges a 15% service fee on all verified savings obtained through our platform. This fee is only charged when we successfully save you money.

User Responsibilities

As a user of our platform, you agree to:

Provide accurate and complete information
Comply with all applicable laws and regulations
Not use the platform for illegal or fraudulent purposes
Maintain the security of your account credentials
Pay all applicable service fees in a timely manner

HIPAA Compliance and Data Protection

Where applicable, we act as a Business Associate under HIPAA and are committed to protecting Protected Health Information (PHI).

Implement appropriate administrative, technical, and physical safeguards
Limit uses and disclosures of PHI to permitted purposes
Report security incidents in accordance with HIPAA requirements
Maintain Business Associate Agreements with applicable Covered Entities

Limitation of Liability

FISIOTOOL SOFTWARE LLC shall not be liable for:

Medical decisions made by healthcare providers
Healthcare outcomes or patient-provider interactions
Advice or treatment provided by independent healthcare professionals
Indirect, incidental, or consequential damages

Use of the platform is at your own risk.

Termination

Either party may terminate these Terms at any time. Upon termination:

Your access to the platform will be discontinued
You will remain responsible for all fees incurred prior to termination
We will return or securely destroy your PHI where feasible

Governing Law

These Terms shall be governed by and interpreted in accordance with the laws of the United States and the State of Wyoming.
